PERSONAL ASSISTANCE PLANS FOR ABBEY PARK RESIDENTS

PLAN A — DAY AND NIGHT ASSISTANCE
• Continental Breakfast delivered to room
• Medication Reminders
• Assistance with dressing/undressing
• Assistance with personal grooming
• Assistance to and from meals
• Two hour check-ins around the clock
• Assistance with two baths per week

PLAN B— DAYTIME ASSISTANCE WITH CHECK-INS
• Continental Breakfast delivered to room
• Medication Reminders
• Assistance with dressing/undressing
• Assistance with personal grooming
• Assistance to and from meals
• Two hour check-ins between 8:00am until 8:00pm
• Assistance with two bath per week
